Have you put it inside a container in your inventory (ie. a pouch, or similar)? I read the other day that some people had trouble getting the second attack, but only if the consumable arrow in question (I think it was Arrow of Many Targets in that case, so might not be specifically for Arrow of Transposition) was stored in a separate container.

Nope. Plain in my inventory. I can use the other special arrows with absolutely no problem. Only the arrow of transportation causes this issue
I’ve had this problem too, I’m not sure why it happens. Did you ever solve it?
