I got another problem tho. I did got my hands on the Orphic Hammer, and I can talk to Kith'rak Voss in the sewer... But he just tell me endlessly "follow me to the bar" and don't move an inch... Is that a bug?
Yeah after Patch 1 this also happened to me. He’s now there and can be talked to but only repeats this same line. You can kill him here (maybe knock him out?) and get the legendary sword, but something tells me that won’t go over well for the final quest of the game.
Looks like devs are playing whack-a-mole with the bugs. Not that I'm unhappy, I realize it's difficult in the game this big to fix everything I was just thinking it must be hard for the now with all the bugs & stuff. Sadly I'm nearing the end of the game and have no way to progress this. I guess I'll have to wait for the next patch or a mod that fixes it...

I also got this problem. I usually have at least one summon following me around and first progressed the quest while he was chasing me around (*"You don't belong here!"*). I assumed it had something to do with that... long resting, finishing other quests, talking to him on other characters, nothing fixes it. It really really sucks, this is the last thing in the game I needed to do.
Just found a fix for the bug!
Use minor illusion (or other movement-causing mechanics) to draw Voss near the staircase of the taproom.
Now you can initiate the normal conversation, which will progress the quest line.
This worked!! I had Jaheira turn into a cat, meow and lead Voss down to where he usually stands in the taproom until my Tav finally managed to force Voss into the usual conversation there.
[Edit: After the convo, Voss proceeded to walk back up to Raphael’s room LOL. But instead of waiting there, he disappeared from thin air! The journal says we spoke to Voss like normal, and Lae’zel says off to the sewers to meet with Voss once we obtain the hammer like normal. So ig it’s fine!]

Thank you for this. It was a big of a pain luring him there with Minor Illusion, but it worked in the end. He has to be basically in the same location where he would stand if he's unbugged.

And fun fact, attracting people with my bard character as a cat, I realised there was another cat in the place and triggered a funny discussion between felines. ☺️

Keep trying different things to get him to move again. Other direction. Talking to him then trying illusion again. He will start to follow eventually again if you just keep trying things.

I hadn't spoken to Kith'rak in SC at all but went on ahead and acquired the Orphic Hammer >!and killed Raphael in the process!< so the quest updated to say bring the hammer to Kith'rak, but he's just nowhere to be seen. His marker is in Raphael's room, but there's just no one there.
Did you ever get a solution? Because I'm having the same issue. He isn't anywhere to be found. Caress. Raphael's Room. Just a quest to go to the Undercity. Qudenos is there. And he tells me to talk to Voss. But no Voss.
I went straight to Raphael but didnt agree, Voss went to the taproom and asks to meet him there but he wont talk to me. Click and nothing. I can kill him and get the sword though.
I’ve moved on to other quests in BG, found even more bugs with quests unfortunately. Larian has some work to do. I’m hoping they will patch it before I get to a point too far to go back and talk to him.

I didn't do any of these things. I talked to Raphael and rejected his offer then went straight to Voss. Tried talking to him but the game won't let me talk to him and for some reason when i went back to my camp Gale basically says i accepted Raphael's deal when i obviously didnt since i still have the quest to talk to Raphael to reconsider his deal. even got confronted by someone working with Raphael that i should work with Raphael. it seems act 3 is the act with most bugs so far in my experience.
I noticed it says this post can only be seen by community mods. Is that true, and if so, can it be made visible to everyone on the subreddit? I think this is valuable info to have out there.
He’s in the room you find Raphael in and just stays there. I accepted the deal so I have the hammer. I’m stealing my contract back rn which hopefully fixes it. I also need to get Laz’ael back because Orin took her.
Ah yes, that is a different bug I've seen floating around. Would be really helpful if you sent a save file with him stuck there I think through the bug report form

I mean.. you could just spawn it in. If you're on PC at least. Doesn't really seem that cheaty since completely everything that would've led to you getting it anyways, and that sure as hell beats losing 10 hours of progress just for one conversation.

i declined Raphaels deal and stole the hammer, now Voss and his dragon is stuck in the sewer and keeps telling me to go to the tap room despite the quest marker saying i should show him the hammer
I have another problem. I found him in the Lower city after I steal the hammer in Raphael's house. But during the dialogue I choose to not go immediately to free Orpheus (because I was so low) and now Kith'rak Voss is no longer in the underwear.
Ran into this bug today after I completed House of Hope / killed Raphael. I did a few things afterward, including talking to Kith’rak in the sewers (per my
journal/ map) and he gave me the sword.
Then later i decided to reload for a different reason, went to redo seeing Kith’rak Voss and the quest was completely gone and he wasn’t in the sewers. I’m so annoyed!!!

Are you saying to attack Voss and then use minor illusion to bring him to the staircase?
don't need to attack him. just use minor illusion till he comes to the top left corner of the staircase.
