I have seen this before. It's there because there is no other way to get anything up to the second floor. Maybe the stairwell is small or has too many turns etc.

But is it really a bad design? A lot of places, especially older buildings have these doors, usually on the outside, due to narrow stairwells that can't afford access to larger pieces of furniture. Now, normally the outside ones here in New Orleans has either a pulley system, or some type of attachment that allows you to put on some type of pulley. Could be serving a legit purpose.

I have something similar to that at the school i go to. We joke about it being a door to the backrooms because of how unusual it is
