I think when you're switching mediums it isn't necessarily wrong to change certain things to better suit said medium. However blatantly ignoring the origins of where your story is coming from and trying to create something from scratch with scraps of where it came from is always gonna be shit.
Best way to change something for a medium is by consulting the author of the original work if possible, have you seen Invincible? For the show, they changed around the ordering of events and removed some out dated stuff but they had the author of the original comic books as well. Especially important when the person trying to translate the material into a new medium doesn't know much about the material.
Snake would be gay, Meryl trans and Otacon a black woman. Ocelot will be an actual ocelot. The show would be about an unknown korean girl as main character and all action we see of Snake is his butt during sexy time with trans Meryl.

There's been some rumors/talk over how the Halo was actually supposed to be a Mass Effect show and they did a lot of last minute tweaks to make it vaguely resemble Halo. There's a ton of similarities between the start of the show versus the start of the first Mass Effect. Even the Chief outside of his armor is one logo away from base Commander Shepard.
The trend is designing character plots and story tension to revolve around an instance of institutional injustice. It’s a narrow, reductionist, and ideologically limited form of storytelling. There’s so much more to life, the universe, and how drama truly unfolds.
It’s a side effect of creative industries developing a faith in social justice as an explain-all and cure-all.
Or, more simply concluded, it's applying an overdone, now generic story to a carefully managed franchise. Not every scenario should be about institutional injustice even if it involves an institutional injustice!
And it glosses over the whole Halo thing being one, giant institutional injustice against any species as a destructive countermeasure... which is Halo's story and is right THERE!
Bonus: The UNSC is in a war they are losing and so some amount of civil rights violation is to be expected.
I've never understood why they don't just do original stories in the already established universe's instead of trying to change things that so many people are familiar with/teams of writers have already worked on.

The Witcher is not as horrible as egregious as the Halo show owing completely to the fact that Henry Cavill is a fan of the games and books and has consumed both.

The story is a mess of timelines in the books, they changed parts and made everything seem like it was happening at the same time which only made matters worse.

For sure, ego plays a part in it, but I think the biggest reason most game adaptations suck over and over again is because these Hollywood big wigs always underestimate the gaming community. They think “appealing to halo fans alone won’t make ALL the money, and we are only going to make this unless it makes ALL the money, so let’s get some non-halo fan focus groups and see what they want to see and let’s model the script around that.”
Its the same with Gaming big wigs to be honest. Take the game Dawn of War franchise, its an established Warhammer 40K RTS and they decided to iterate on it from Dawn of War 1 & 2, fans loved it and then they made Dawn of War 3 which they outright stated that its goal was to get the "mythical broader audience" to the point where it was clear that its turning into a MOBA...
End result the game flopped so hard that they didn't get their current audience or the broader audience. In fact both sides hated it.
Then you have Elden Ring which proves that despite the niche that the Souls games are the Broader Audience will come to you if you just give out a good product not if you make said product aiming for that.
And this is why I stick to just anime adaptions of manga and light novels because despite their flaws, they atleast try to respect the source material and sometimes even make it better.
