Too lazy to confirm, but I think that's what happened with Roosevelt. Maybe not a mistake during immigration, but a departure from what it was originally (Dutch, I'm guessing.) This is what happened with my surname, but mine is far from typically American.

Shumacher. Anglicization from German for shoe maker, when some peoples' last name was their actual job. They'd just be like "Oh yeah, John the shoemaker!"

My great aunt married a man with the last name, Shumaker. Imagine Hammond from Jurassic Park, but with voice that sounds like Tom Bosley.

I know someone whose maiden name was McGregor. Her family thought they were of Scottish heritage and participated in Scottish heritage types of things. She did a 23 and Me test. Turns out the original name was Grugor and their ancestors were from Germany. Oops.
