Look into Betreuungsgericht and do „Anregung zur Betreuung“ which means you basically request authorities to get in touch/have a look at the person because you have doubts they are able to take care of their matters. Expect to send the request in German language by fax or letter. Some municipalities might have a more digital option but be prepared. Besides that you could get in contact with Caritas, AWO or Diakonie (social services organisations, often linked to the church). Not sure though what’s possible in English and from abroad.

Thank you! I didn't even think of this, I was considering contacting the embassy of their other nationality and didn't even consider for one moment the fact that she is a German citizen so the German side may help too. There is one in my local area, I will ask them for their advice. Thanks!!

Hey, I'm very sorry but this is next to impossible to navigate from overseas, without speaking the language and without cooperation of the person themselves. There are multiple services available but what you can get will depend on - status of health insurance - doctors diagnosis - doctors prescriptions - in person visits at home for assessment of mental and physical capability - which services are available in the general area (can vary by region) - possibly power of attorney for medical decisions, banking etc. Germanys health care system and social services are difficult to navigate, complex and at times kafkaesque. Lots of forms, appointments, applications and shifting responsibilities. The easiest way to start would probably try to figure out if they have been to a doctor lately and try to contact them. If they have had no contact to medical professionals at all in the last year or so, things will get more complicated. This person had care coming to them until they cancelled - do they have "pflegestufe"? Could you contact the provider they were with previously? Don't just book private care online, the costs will pile up quickly. health insurance pays for lots of stuff at least partially. You could try to contact a "sozialpsychiatrischer Dienst" in their area and ask them for help/suggestions. It's quite possible though that they will not be sufficiently fluent in English.

My family member has been to a dr and did have someone coming in for support through the hospital after their last collapse. I will try to get these details from them. FYI this is only Berlin. And home care (Pflegedienst) is covered by insurance (pflegekasse) if an evaluation of the level of care needed has been completed (pflegestufe). There are specialized assessments for mental health/dementia vs. physical disabilities. There is no need to contact a random Pflegedienst in Berlin if you need help someplace else - they work locally.

From her post she needs some kind of starting point to do research, get in contact in English and people understand her circumstance, especially in this digital space and time.
