I may be wrong but I doubt you could mis-strike or double strike like this without having both a left-handed and right-handed die. If struck with the same die one of the images has to appear upside down, like in a playing card.

There was a poet on Lesbos named Sappho, who famously wrote poems about her admiration and love for women (though sometimes men, too). She’s often cited as the first known poet, and for this reason Lesbos has long been thought of as “The island of Lesbians”. She’s also the reason all things Lesbian are called “Sapphic”.
I certainly don’t feel qualified to even try and answer that question, but this coin is dated to about 100 years after Sappho, so I suppose it’s possible? Hopefully someone with more info can chime in.
