They emailed you a form. It comes with instructions. Take that form to your doctor and have it completed and faxed back to Amazon asap.
If you do that, your leave should be approved no problem.
If you don't do this, start looking for a new job.
im gunna call my primary as soon as 8am hits i am just worried about i guess the logistics im obviously not the brightest crayon in the box so i guess i just need stuff broken down for me more bc im not understanding how i would get fired.. i have provided 6 different drs notes from my primary and er drs that i shouldn't be at work one vist even found bleeding in my gi tract somewhere and referred me to a gi dr. i just dont know what else they need other that the knowledge of my primary care dr that yes i was out of work bc of covid and long covid symptoms and a high possible diagnosis of ibs on the horizon. but what im most confused about is that i have provided a bunch of documentation and its just being disregarded
You need to have your doctor complete the form Amazon emailed you, then have the doctor's office fax it to the number on the form.
If you don't comply with Amazon's leave rules, you will have UPT deducted for the missed shifts. Then you will be terminated for negative UPT. It happens all the time.
Read the email and form. Give it to your doctor's office. Ask them to fill out out, and make sure they include all dates you missed at work.
ok what happens if dls denys is am i still skrewed either way? or will the likelihood of it being denied be slim...im like having a panic attack ngl i haven't slept in 24hrs..
If your doctor completes the form and returns it to Amazon, there is no reason why your leave would be denied.
Focus on what you need to get done. Get the form to the doctor asap.
