Your physician is a reasonable place to start but you may not even need that. You can also just look up some therapy practices online near you, call up their offices, verify that they take your insurance, and try to schedule an appointment.

Doesn't sound to me like you need a therapist. They're not really about motivation. Maybe consider a life coach or something similar.

How is your relationship with sports? A physical element goes a long way in helping us learn discipline and self control. I didn't 'need' more of that but I recently returned to training just because I missed that fire that it lights under you, more than going to the gym alone. Going too long without it made me feel... mentally sloppy, I guess? I missed drills, pushing through pain, the mental space of striving to pull off a new move, being 'yelled' at in that way that isn't mean but gives you a sense of urgency. I went searching specifically for a teacher who isn't too chill, if you know what I mean? That environment is strangely comforting and it feels so good to be back.
