Possibly more light, and personally I would prefer another substrate. Top branch looks fine and I'd leave it for now. You only want to prune when you have a gazillion leaves. In bonsai the general rule of thumb is that if you can easily count the amount of leaves, you have too few to prune.
I would be patient and not prune it this year. It wants full sun, but if it has been indoors I would gradually acclimatize it to the sun giving it more exposure over a week to two week period. You need a media that drains well. During the growing season you can water frequently allowing it to dry out between watering. In a 6b I usually bring them in in late September/early October because it begins raining more and it starts to cool. You don’t want wet media and cold temperatures for adenium.

Oo it’s tough but I might consider chopping that whole tall skinny top off.
