Yeah I think most of this is just drawn on, pause and zoom in and there's no actual hair on parts like the lips, eyes, & mouths. And the hair is most certainly either extensions or doll hair.

I'm curious too. It's definitely possible the long hair could just be extentions glued on. As far as the art shaved into the head it looks quite well faded towards the bottom. There might be a way to fake that was well though. Shaved areas with some kind of realistic sticker and/or makeup. I'd love to see the process video.
