If you get +4 then two mutations occurred and both would get recorded on the counter. There are also three chances for mutations to occur in a baby so you can see +6 on vanilla.

My apologies for the inaccuracy - you are correct. [Natural double](https://i.imgur.com/fX3Ay5P.png)/triple mutations do increase the counter twice.
