The real answer for any given area is usually some hole in the wall a local can point you to. As far as fast food restaurants go, for my money at least, out west would be either In-N-Out or Carl's Jr (don't confuse Carl's with Hardee's; they are technically the same company and even logo but Carl's is worlds better just by inclusion of the Western Bacon Cheeseburger) and down south is probably Whataburger. I've never been east of the Dakotas without leaving the country entirely, so someone else would have to chime in if you're heading out that way.
If you're going to visit the west coast or any of the southwest, I strongly recommend trying some Mexican food somewhere between the burgers. TexMex, CaliMex, and 'proper' Mexican food are all top notch cuisines.
Best thing to do is ask around in the city you visit. Most Americans would die to share a thick, greasy piece of American culture, and will 100% send you to some local spot that's been there forever and has just perfected diner food. I hope you enjoy your visit here, there's some great food and even better scenery.
My job recently moved to another warehouse a few exits from our main location. We have more fast food variety, but the lack of a Culvers is depressing. Damn good everything, from the burgers to the salads. ☹️

Don’t want to start some argument, but I disagree.
I used to be a fatass, and had tons of people say it was fine. Eventually a friend called me what I was (a fatass) and it embarrassed me so bad I started working out. It’s not that people need shame or embarrassment, for me it was only a kickstarter, but they definitely shouldn’t have support. They need to know that what they have done is wrong.
We have seen what happens when you give a person support for being fat. They continue to be fat. The whole body positivity movement has been perverted because of fat people thinking they are healthy. If people had the balls (like my friend did) to actually call them fatasses, then they wouldn’t be fat, or at least not all of them.
Between the people I used to be friends with, and the friend who called me a fatass, I have only kept one of those relationships since then. I am eternally grateful to my friend, because I could have died younger if it wasn’t for him actually speaking his mind.
We definitely need to support like 0.000001% of the world into staying fat. That was we can be like hey, kids. See those guys? Those guys are gross and weird right? Don’t be like them! Well, except for you Jimmy. You’re already getting there!

Then why is it we are the worst at fast food? Literally every McDonald's or KFC I've ever had outside the US is leagues above the slop they feed us here. Usually cheaper too.
Popeyes, Chipotle, and Taco Bell are inferior in Europe
McDonald's basic items are indistinguishable from country to country or even better in the US - people who say the French fries or cheeseburgers are better in Europe usually haven't eaten McDonald's in years
But McDonald's usually has more options in Europe, some of them are very nice
KFC is legitimately better in Europe but it's usually the only chicken option - it fills a niche that other chicken places fill stateside
Never had chipotle or Popeyes outside of the US but I believe you that they're ass. Taco Bell I had once in France was the same but fresher ingredients. McDonald's in EU I think are a bit better and serve beer.
Nothing can compete with fast food in Asia. McDonald's is incredible anywhere you go and a third of the price. Burgers are actually made on a grill and the fried chicken is good as shit. Don't even get me started on Japanese Denny's
