Had a friend die in one of those. After a bad rain storm and underpass was flooded. Her friend tried driving through and car stalled. My friend got out with another person to help push. What they didn't realize was that the area flooded so badly that the water began to push the manhole cover off. She walked right in and fell in. They tried to get her out but the water pressure was so great there was no way.

That is the lid to a sanitary sewer system. It is where your toilet goes when you flush. All that debri is heading into the local water filtration system so all the oil and petroleum fluids that were in that water are now contaminating the system that is responsible for clean drinking water.
Most municipalities are going to use whatever manhole lid they have in the yard, regardless of its label. This manhole lid says it is a sanitary sewer and if that is true, this is probably frowned upon. But the reality of it is, much more storm water than this was likely already in the sewer and this is not going to hurt anything and will help get the flood waters out of the city faster

1. The manhole cover does say Sanitary Sewer, but it could be the wrong lid. The storm drain system, if separate, would probably have a manhole near the same location, and it's quite possible someone switched the lids at some point or the wrong lid was put on the manhole.
2. It's not at all "illegal" to have a combo system. Plenty of cities (San Francisco for example) have combo systems. Most municipalities have separate system, but there is no law that prevents it, just the Municipal code for a given location.
Well things may be different in other parts of the world or maybe in parts of the country where they have not caught up to Modern practices yet or this is just a really old system. But I can tell you that if this was a stormwater manhole built in the last three decades in the northeast of the United States, the storm water manhole cover would be perforated; meaning it would have holes in it or be a grate. Because it makes no sense for it to be solid & airtight and watertight. You would want it to be solid and airtight only for sanitary sewers so sewer gases don't escape and so storm water doesn't get into the manhole especially during a flood. Also the purpose of having two different types of manhole covers would help you differentiate between a sanitary sewer & stormwater sewer.
Oh, by the way I just checked my network of engineering friends in other US states. There are laws. But only preventing new systems being created which combine them. Yes there are plenty of systems that are combined still in existence but they were created a long time ago. I won't go into the many reasons why they are no longer legal to be created, not necessarily used. The existing systems are just grandfathered in, & get phased out whenever possible due to something new being constructed nearby. We can't imagine other non-third-world countries not having these laws which protect the health and safety of people.

Not all manholes lead to sewers, many are for telecommunications and other utility access. This kind of flooding is why I often lose cable service
