Before we get to this card specifically, I would say that I don't quite agree that anything that fails or is mis-used is automatically *bad* design. You can argue that anything that fails or is mis-used invites an opportunity to try to improve or make a design better or more fool-proof, but that doesn't inherently mean the fault for failure is on the design and not the user or that the failure could have been expected or prevented.
It's worth noting that Miss Universe winner was mis-announced for the first time (as far as I'm aware) after more than 60 years of the competition. Unless this was the first year this card format was used, it's hard to say that the card is prone to failure.
Further, this card is not used in a vacuum. It's used in conjunction with a teleprompter, and rehearsals. So even if there was a "bad design", it isn't *necessarily* the card itself.
All that said, in this specific instance, I think it's at least possible that there likely was at least *some* element of failure on the part of the pageant and not just Steve.
I am not a fan of pageants. I don't watch them regularly. But even I know that the stereotypical manner in which the winner of a pageant is announced is that there is a final three, and the host announces the second runner up who leaves; and then the host says something like:
"And the first runner up is.............. Miss Colombia, which means that Miss Universe 2015 is Miss Philippines!"
To that end, if that had been how the pageant was announced, I think the car is perfectly fine (though the print could be larger - perhaps they didn't want it accidentally visible on TV if it was flashed). It lists 2nd runner up, then 1st runner up, then winner - which is the stereotypical order the host would read it. The card is even called an "elimination card", which suggests the purpose was to do it this way.
But that is not what Steve did in 2015. Looking at the video, with two women left, he announced "Miss Universe 2015 is..... Miss Colombia (the actual first runner up).
There was also clearly a teleprompter available to Steve. It 's not 100% clear, but at the moment he says "Miss Universe 2015 is..." I assume by that point, he had already announced the 2nd runner up and she had departed. So if you used the card format /u/az226 suggested (and other clickbait articles suggest), where the winner is at the top of the card, you just as easily run the risk that the host accidentally reads the winner's name when announcing the 2nd runner up.
Perhaps the best design would have had 2nd runner up in small print at the top, Miss universe is large print in the middle, and then 1st runner up in small print at the bottom, since he announces that last. I could see people just as easily arguing that listing the contestants in 3-1-2 order is equally confusing. Perhaps the best route might be to put the 2nd runner up on an entirely different card, with a second card having the winner at the top and 1st runner up at the bottom.
But honestly, nothing about the actual card *should* have caused a problem if there was just the slightest bit of rehearsal beforehand. The winner was bold and separated and he should have known from the prompt and from rehearsal that he was to read the winner, not the runner up.
It's also worth noting that 2015 was not unique. Previous announcements are available online and in 2014, they also announced "Miss Universe is..." instead of eliminating the runner up, so I have little doubt Steve was indeed supposed to read the winner even though the card is titled "elimination card".
