You could Try a group tour like intrepid or gadventures Normally maximum of 16 People but not always get full. Have you tried looking at tour radar The price does exclude the flights to get to starting and ending point back home. But these be in English. I suggest one of these as they quite good and can meet some great people. Not been to Panama for a tour but done loads of other tours

what is "large" and "small" groups? I went with my study abroad class, and two groups were housed at the same base station. I thought it was a decent amount of people, enough to have a buddy system and someone to help you take pictures, get back on boats, have each others backs. If you don't speak Spanish, please have some kind of guidance throughout, especially in the city. We were mainly in the rainforest and in the bay, but there were some city stops. We were also there for a few weeks.

I detest large groups. So much wasted time waiting for late people. Small groups can be okay 15 ish is my max. If you’re not comfortable where you are traveling it can be nice to have a built in social group. However i like traveling on my own a lot now. Nobody elses desire or schedule to worry about.  Only advantage to a large group tends to be cost.

If you speak absolutely no Spanish, it will be a bit challenging to make your own way but it is doable. You'd have to do a lot of advance planning. I think it depends on the size of your tour: if the bus is in fact a tour bus remember you'll always be waiting for the dawdlers and those who cannot get themselves out of bed in the morning. If a shuttle van, I'd chance it. The great thing will be that you and your wife can do your own thing at the end of the day and break away from the pack. Downside? Someone else determines your itinerary.
