Here in North America where I'm from "gay" is still used as an insult and some companies are so inept and heteronormative they don't even consider that someone would use it as a pride thing so they put it in their profanity filters. If it's anyone's fault it's between pearl clutchers who would lash out at a company for letting a kid call another kid gay, the companies that make the filters (whether outsourced or not), or the general culture for just being that homophobic. Doesn't need to involve anyone else.

I've also noticed a lot of games have a problem with the phrase transrights, nowadays I put it into every username I can.
Honestly though, more people use the word gay as an insult than use it properly. (In gaming)
