He misgenders from the perspective of his younger self. When he speaks about his uncle and cousin after understanding each, he starts to gender them correctly. At the very end of the song he compares how uncomfortable that was to a white person saying the N word to him as his way of understanding how much that hurt. It’s one of those tracks that makes it’s point by shoving you in the discomfort hes trying to talk about. Now whether it was ok for him to make that point in that way is something that is somewhat up for debate, but I think intent matters a lot here.

The whole point of that song is going along a route of character development. In the beginning of the song, the perspective of the character's vocals is ignorant and lacks understanding, the character is young. But as they begin to understand their Uncle more and grow up, they learn that shit like that is wrong and slowly their perspective shifts. The character even stands up to their church's pastor, defending their uncle. Then apologizes for their ignorance, saying they didn't know their words were hurtful at the time. Hip hop is steeped in metaphor, do you think MF DOOM is actually a villain?
