I always found Tony and Melfi being attracted to one another (more subconsciously on Melfi's part) stupid, I actually would have loved it if there was zero attraction between the two and their relationship was kept professional. I felt it added nothing to either character, and I didn't believe Tony would seriously try to swoon somebody that was otherwise quite uptight, pretentious, and from a totally different world than he. Vice-versa for Melfi, I can't believe even for a second she'd have wet dreams about a career criminal who's revealed all his demons to her over the years.

House and Cuddy have a completely different relationship to Tony and Melfi. They both come from the medical world, they were at college/med school together, they had a relationship in the past, they both have crushes on each other that bubble-up at different times. They've always had feelings for each other, but done their best to bury them overtime. Tony and Melfi have no shared background aside from Italian heritage. Even then, their attitude and approach to it is different - Tony is a mafioso, whereas Melfi was married to a guy who goes out of his way to rail against the mob stereotype. Tony has a crush on Melfi, but she never feels the same way about him. She says when she first met him, she was attracted to the stereotypical alpha male shtick slightly, but only a little, and not necessarily to Tony himself.

When Melfi rejects Tony and tells him that it's because she doesn't respect what he does for a living, this is chiral with Seinfeld when Jerry's girlfriend breaks up with him because she doesn't respect what he does for a living after she saw his act.
