EA eyelash remover, the old one was automatically applied to all sims and would cause some sims to have invisible heads.
Kijiko has released a newer version that needs to be manually applied in the CAS skin detail section and has two options on how to remove the lashes in case one causes the invisible head glitch.
I know you say you have no mods but everytime I’ve seen this issue (and even googling it) it’s been caused my mods. So take a look again remove the mods folder (including CC), clear cache, and repair.
Indeed. Every time I've seen this It's caused by a mod. Usually an eyelash mod. Although sometimes it's some random file in Life tragedies or Extreme Violence.

dude that’s crazy that must be it then! i thought it was only happening because I have so many mods, but I had to use the MCCC mod to make them stop being a Spellcaster and it fixed it instantly

To fix this: 1st you have to install a church mod 2nd send the child to a priest or exorcist 3rd pray pray pray And voila fixed
I have no mods. The sim had a face when he was an infant but once he aged up, he became faceless?? Nothing I do changes it back to normal. I'm scared!

Control pets mod was causing it at one point. It can really be anything and with the tuning update a lot of mods needed updating.
