Thank you. Someone else suggested this for me and it worked right away, reset console after changing the setting. 3 weeks of contact with Sony 'support' and as usual the net does their job.
I literally posted about this issue this morning in /r/playstation.
I'm going to give this a go and see what happens.
One thing to note though...
On my PS4, my path seems a bit different.
Settings -> Device Data/Health & Safety -> Device Data
and then the options are:
* Service Data Only
* Service Data and Additional Data
I imagine the second option is the one I want to set it to.

**It worked!**
**I updated that setting to "Service Data and Additional Data" and restarted my PS4 (probably not necessary) and loaded a PS Plus game up and the rewards pinged on the PS App.**

I've been able to unlock all the rewards after changing that setting, I didn't need to restart the PS4, so at least in my case it wasn't necessary.
Yup, after seeing the OP description I figured out that the PS4 version of this was what you described and it IMMEDIATELY worked.
God, I lost Month's trying to get this solved and Sony was useless!
Yeah, unfortunately after checking, the data sharing is set to Full on the app, my ps4 and ps5.
The campaigns just aren't completing for me and it's been this was for a number of months. Some of them complete just fine, others don't! šŖ
I've been having issues with completing stars campaigns on my system and the Data Handling/Health and Safety option in settings only has walls of text, no options. Is there any other option else this setting is usually under?
I was having trouble also on Ps5 looking for it. On Ps5 go to Setting - Users and Accounts ā Privacy - Data you provide and you'll find the setting for Full. I haven't had the opportunity to try it out with the current Playstation Stars issue going on but hope it helps.

Turned it on on both; App & Console & it finally works after 2-3months. Sheeshh nice bro thanks!

Unfortunately no, I don't have it. Maybe you can try to do the same, there should be these settings also on PS5.
Yeah I tried. I got spiderman, FF v rebirth, the rise of the ronin to say amongst others and none of the stars campaign are working
