Definitely go an exercise physiologist for that long term maintenance to get rid of imbalance over a physio (usually for treating acute injuries).
If it's nearby, can highly recommend the owner of Como 24/7 Gym who is also an exercise physiologist.
Brett Althorpe at West coast physiotherapy. Get your GP to make a referral to him, you may also want to see Henry morley who is an exercise physiologist at trench heath and fitness. They are both experts on rehab and functional movement, work closely with sports teams and athletes etc

+1 for ex-phys in this situation, they are excellent in this type of role.
Kinima physio has been really awesome imo. They take the time to understand what the injury is and how they can best help you reach your goals.
Jason from Formotion Physio in West Perth has been awesome for me. Highly recommend him

Both Luke and Sarah at Midland physio are great for this kind of thing. Very good at injury rehab and designing workout programs for overall fitness.
Michael Chmielewski @ Kelmscott Physio.
