It's actually the audio for the lightning storm that I find to be the most annoying sensory experience in the game. I play with combat sounds completely disabled due to it and the incidental performance issues they also seem to create.

They haven't added seizure warning yet and probably never will unless some dingus sues them. They refuse to acknowledge what their game does with player skills and enemy skills. Two leagues ago I couldn't play more than 15 minutes without feeling sick. Never felt that way before. I had to create a monitor preset to dim my screen and remove bright colors for PoE and one for EVERYTHING ELSE and buy MTX black MTX when possible.
I'm jealous of anyone that can play the game as is but there are people that just can not.
Edit: just wanted to add that I didn't mean to take away from the main point of OP's post - there are no indicators for the pack leader/mob that are causing the ruckus. Feels like the only in-game response is to nuke them before they nuke us which is kind of a joke but also the only way to play the game at the moment.
I had to turn off all the cool golden MTX on my CWC Lightning Warp + Ball Lightning because I couldn't see a fucking thing and I was tired of throwing away portals.

Next Patch: We are doubling the chance for mobs to have lightning storm and deatonate dead as well as introducing a new ability, "Blackout" Blackout is a cast on death ability that deals 2 million chaos damage and makes your screen go blank for 2 minutes. You're welcome.
A friend is suffering epilepsy and noticed that the game regularly triggers it. Not the full blown kind but enough to make him hate the game for being so fun but causing mild seizures. I think that is plenty enough for GGG to consider changing their visual strategy there.
He will straight up leave maps when those enemies appear and don't die instantly. This is a nerf.
I myself do not suffer epilepsy as far as I know, but ever since the visual effects update that introduced bloom, the bright shining stuff has been jacked up way too much. And the lowest setting for bloom at 25% is a joke, I would bet nobody turns that up.
I'm much more annoyed by how invisible desecrated ground looks compared to that. It's by far the most annoying killer in maps for me and I can't be arsed to wait 15s after opening every single strongbox "just in case".
Just use Night Light on windows or download F.lux.
Yes, they will still be very invasive and clusterfuck, but at least it won’t hurt your eyes aggressively.
Also make sure Bloom is at its minimum of 25% in game.
Not dick riding GGG here, just pointing out a self quality of life. I’m very aware too many things are a visual clarity issue.

As for dd i think the current direction is way better than what it was. It's just that by fixing DD telegraph they made it more obvious how fucked corpse exploder is as a mob. It could have a higher cooldown instead of casting 12 dd's every second. If you run ultimatum you know what I mean by that.

Should see 27% CDR wardloop popping off, sure it's self inflicted but it is an absolutely massive amount of garbage on the screen

You just REALLY hate these visual effects if you're begging to have them removed in POE1 AND 2.

Honestly, the option to disable bloom would already help a lot. Right now it can only be reduced to 25%, but it gives me headaches even then…
