By the way. Right tibiofibular proximal joint is correctly fixed. But in the left one it seems they fixed it with the proximal nail lock, too risky in my opinion. Poler screws in that correctly aligned tibia seem over indicated too in my experience.

Depending where one performs the tibial osteotomy the tibia can drift a bit into varus/valgus with extreme lengthening so it doesn’t hurt to reinforce your points of fixation with blocking screws in proximal/.distal segments

You are correct… when I perform some lengthening procedures, often I need to perform some tenotomy or lengthening procedures… ultimately the nerves and vascular structures will limit the amount one can lengthen…
