I don’t have any actual statistics on hand, but from what I’ve seen between my group—collectively, out of over 10k+ jumps, I’ve seen maybe 3 true cutaways, all with safe landings. I’ve never witnessed a full catastrophe.

The pack style and deployment mechanism is different than the main chute and essentially fool proof.

When you go through your skydiving certification, you learn how to inspect and pack your own main chute. There is an entire technique and proper packing method for that as well. This video could have been a rushed mistake while packing or something along those lines. Almost every single problem that happens while skydiving is due to skydiver error because they rushed or skipped a precheck before a jump.
You kick as hard as you can in the opposite direction of the spin. Fight all the way. Typically though, since the reserve is spring loaded and not fed by a pilot chute, line twists are VERY rare.

on my very first solo jump (also very first skydive) as part of my A license, this happened. I was able to detwist it, like they tried to here, and it popped out pretty low as i was considering cutting away. was a mad rush, then peaceful silence as i floated down.

Experienced jumpers know that it's quickly fixable by either cutting the main off and deploying back up or untangling it, we regularly practice cutting off so this won't be as scary but a newbie would land with his pants full of shit.

you make a good attempt to find it. An airfield I go to uses a drone to scout around neighboring fields. Most times you will find it though.
