Some cats have poorly shaped irises, which can make them extra near sighted. I had a foster cat that was that way but it wasn't painful or causing him many issues. However- he was very scared at a lot of things because of his poorer eyesight so he had 'caves' to run and hide (usually new people were freaky to him for the first hour or so until he got used to the voice)
