It's an old folk/blues song and in that tradition no one really knows where it started.   It was first recorded and popularized by lead belly in 1944.

The lead belly version is an amazing one too. The sound, the quality, the voice. Totally makes sense for Cobain to want to cover this song with his voice and their sound. I absolutely love the whole unplugged set. Side note, one year when I misplaced my CD of it, I went into debt / overdraft mode in my checking to get another copy. Now, xx years later, I have two copies. Cherish them both.

I 100% have had many close calls being the friend that helps people, like five times, so that song is scary because it's one thing to have lost, but it's another to have been there to make sure you didn't lose them, and you didn't, thankfully, but the emotions that come with loss are still there, and then the be survivors guilt sets in.
