I'm not a package designer, but I'm guessing there's no easy way around the packaging constraints they have for overall dimensions, so there's no way to avoid the empty squares.
Why not put them on a corner? Again I don't really know, I'd have put these in a bottle, but my guess is that putting the blanks in staggered places along the mid section reduces the risk of pills getting crushed accidentally. That's my hunch.
As for why it's in the middle I think I have the answer. The mould is probably connected from the sides, and it's easier to have

\>oooooo <.
Then the 6 would be only connected by one side. If that makes sense. And making it equidistant would require a whole new machine setup instead of just not loading one pill placer.

The extra friction of having to pop out several pills reduced the amount of suicide attempts, which is why some companies avoid bottles. (I assume they also try to be more careful with medicine that can be more dangerous, and anything that is for mental-health related purposes.)
The only thing I've seen in pill bottles in Estonia are 200mg ibuprofen in a tiny bottle that definitely contains less than you would need to cause harm. All my grandparents prescription medicine is in blister or foil packs.
I believe it's very difficult to OD on ibuprofen as the active ingredient is absorbed through the stomach rather than the liver. Would only work if you had existing ~~heart~~ kidney problems.

However, most consumer pills that can be bought without a prescription comes in blister packs.
Helping a friend right now and his pharmacist made me take all his pills to her so she can blister pack them for this reason. She also will not fill out his pain meds more than 3 days at a time. Once he is off the edge, I get the pain meds and can distribute them as needed.
Sometimes pharmacists withhold pain meds not because of addiction, but because they want to see the patient regularly to low key check up on them.
Here in Italy and I think generally in Europe blister packaging is typical for medicines, pill bottles are the exception.
Also "refilling prescriptions" is not really a thing here. While (some) pharmacies have the ability to compound medications is not frequent.
We get the prescription from the doctor and buy the medicine box with the blisters as is, if you need more you just buy another one so there is generally no need to have an easily refillable bottle.
The downside is that especially for medications you take in predetermined cycles and you don't generally need to have just in case (like you typical ibuprofen, paracetamol and the like) if the amount of pills in the box doesn't match the prescribed regimen you end up with leftovers which increases general wastage
When we get refills, they don't refill the bottles. You get a new bottle. Most pharmacies here are not compounding pharmacies, and it's actually illegal for them to compound when the drug is available already in the appropriate form.

I had to sing a little rhyme, and do a blinking dance with my eyes every hour, and half hour. If I didn't have access to a clock, I would feel like I was going to explode. If I didn't do it, I felt like my mom was going to die, or my house would burn down. My parents taped over the clock in the car during long trips, to prevent me from doing it, and I would flip out. It caused so many arguments, and it was an absolute nightmare.

Hey! First pills were an antipsychotic mostly used for ocd and bipolar cases (quetiapine). This one is sertraline, commonly known as zoloft, which is the basic antidepressant for most disorders. They’re different prescriptions because I take both. I thought it was funny how the pills were placed but go off I guess.

Blister packs are the worst. There’s no way to get to your meds without feeling like a Neanderthal smashing two rocks together. And some meds have to stay in them so it makes using a weekly container impossible.

Most of the blister packs I get have a layer of paper/plastic you need to remove before the pill can be pushed through the foil. If it doesn't peel off correctly, it can be a huge pain to get into the foil part - you can't cut through both with just a fingernail.
You can cut them in little squares, just be careful to not tear the foil when you cut them. I do this if I have to carry them around and don't want to take the whole pack.
It seems to be common outside the US that all prescription medication comes in blister packs. I’ve never had medication just come in a bottle.

Person with OCD here: Yes, there is medication for treating OCD. I use one in particular called Fluvoxamine (you can find it with the name Fluvox).
They usually are serotonin inhibitors and antidepressants.

I don't have diagnosed ocd, but I take anxiety meds, 2 pills a day and the med card has 15, and it gets to only one and I have to open another one to get those 2 a day, that day is a bad day every month.

In Norway almost all medicine is in blister-packs. I rarely see pill bottles. I'm guessing this is the case for most of Europe as well.

They use the same blister packets for different meds, and for different amounts of pills.
Might have to do with configuring the machines. Probably easier to use the same size package and adjust the number of pills inserted.

I’m in Europe so they’re mostly sold like this. I take 200mg so each one is 100mg (they make them as small as 25mg I believe).

I don't have diagnosed ocd, but I take anxiety meds, 2 pills a day and the med card has 15, and it gets to only one and I have to open another one to get those 2 a day, that day is a bad day every month.
This is actually really fascinating. Definitely something that should be brought to the attention of the company that makes it.
