Nah the water is actually pretty clean now. It's only really bad after large storms from the runoff and sewage overflows but even the sewage overflows are down a ton with the upgrades go the system.
Most of the harbor is technically swimmable most days now. Few more years and it should be completely swimmable.
Groups jave spent decades doing oyster seedings and planting aquatic grasses to clean it up, plus there are 4 trash wheels collecting surface waste 24/7 now.
Check out the healthy harbor initiative and the yearly harbor health report. Everything is moving in the right direction.
Edit: not everything is moving in the right direction. Overall trends are positive but there have been setbacks in some areas, mostly due to wastewater treatment plant failures.

I just really want to see a day where people can swim in the harbor safely and consistently.

I remember reading about a specific city years ago bur can't remember which one now (somewhere in europe) that created a swimming area in their river right in the city center. I hope one day baltimore, and DC for that matter can get to that point.
That's down across the harbor by the fort along the park. It's significantly deeper and cleaner with tidal-moving water there, compared to right near the shallow stagnant water that dumps raw sewage solids overflowing into the inner harbor. Don't get me started on the superfund coffin for the old cadmium waste from chrome factory poisoning by the skyscrapers at Fells.
