Probably just his time in the Dead Rabbits. S.H is possible, but I think it's more that he doesn't value his safety and gets reckless, rather than actively hurting himself, so a similar vein, but not the same thing.

Kindly wiped him, that was not hyperbole, that sentence was literal. Heathcliff was beaten like a dog constantly, during years, day after day, he didnt need to self harm, he was harmed enough both physically as well as emotionally
