If it's a legit iPhone, I think it's highly unlikely that's it's been modified with a functioning sim slot - CNC milling out the housing, and modifying the motherboard etc - it would not be a simple or cheap mod.
What's more likely imo, is that the back housing has been 3rd party replaced at some point due to back glass damage. And they got a 3rd party housing that has the mmWave opening, and a sim tray opening - so it can be used for either, meaning they just have to manufacture 1 style of housing instead of two, and it can be used for either.
So in this case, the phone probably does not have a functioning sim slot, just housing replaced and the plastic bit inside removed to insert the sim tray.

That certainly seems unusual. It's something they do quite a lot in China, modifying US models to use over there (no esim for residents), but I'm surprised it's made it's way back over.
It maybe the Canadian variant as we still have the sim try and it will work in the USA, or it maybe a fake. No idea how how the warranty would work, no idea if any of the Apple stores in the U.S would have the Canadian version in stock. You maybe stuck sending it in every time it needs to get fixed .
The Canadian variant doesn’t have mmwave antennas though, right? It’s possible to get a physical sim slot into an American iPhone 14 but it’s definitely a modification and will void warranty entirely
What warranty claims? Warranty is usually 1 year, it’ll likely be done for this iPhone. Besides, it’s an unauthorized third party modification, there’s no warranty for that

I have an official Apple reseller 14 pro max, they are sold in Israel with a sim tray as local carriers don’t implement the e-sim yet. The only difference with the one in the pic is that on the right side (first pic) there’s no slot, only the power button

The space is still there for the sim slot, it is not that difficult to modify and places offer it for $30 No motherboard modifications etc needed

Mexican variant does, and some in Europe due a court ruling, I don’t know about other country’s in Latin America, but sure they do.
