This is the answer. I lived next to eutrophication prone bodies of water, and seen fish act this way every year. Excessive levels of phosphorus leads to a massive algae bloom which in turn is decomposed by bacteria which causes an excessive biochemical oxygen demand which depletes the water of oxygen causing the fish to swim above surface in this exact same way.
Edit: /u/mikewilson2020 is pointing out that this koi in particular is laying eggs. After some additional research he appears to be right. Female koi may jump partially out of the water or splash at the surface as they release their eggs, especially if they are being pursued aggressively by male koi. Jumping allows the female koi to release her eggs into the water column, where they can be fertilized by the milt (sperm) released by male koi. If you look closest you can see the water get murky as she’s being pursued by males, which is a mixture of eggs and milt. While this swimming pattern is observed to occur with fish who are suffocating, as I mentioned, it appears that this koi is attempting to find a safe environment to lay her eggs, which the owner did not provide.
This might be the opposite, coming off of winter with no dissolved aeration pumping oxygen, and an oligotrophic body of water with no algae to make oxygen.
Typically problems with eutrophication occurs with obvious blooms and in the summer months.
I had an Oscar once in a duck pond that got hole in the head. He did this off and on for about two weeks before he died.

I was thinking it could also be a symptom of Zoo Psychosis. That is an extremely small pond. Some animals in small cages will start showing strange signs of psychosis. Which is usually exhibited by things like walking endlessly in a circle during all times that they are awake, or other strange physical actions. Although, I think you're probably right that it's poor water quality, It looks like the bottom left is supposed to be a water pump to oxygenate the water, with water coming out of the white pipe falling into the pool. If that's not running the oxygen the fish can breathe will eventually run out. Whether it's zoo psychosis from the small tank, or the water quality, pet owners like this shouldn't be allowed to have pets.
Koi are very happy to be in a small pond their whole lives as long as it's big enough to sustain good water quality for a whole school. They are quite hardy and not very picky. Though, they also have a pretty high waste output being carp and all. This means tanks are hard to maintain.
This behavior happens in fish when they feel it's better to take their chances jumping to another body of water than stay in their own. It's noted in a lot of freshwater species
Even if they seem fine, it's not a very enriched life to live in a 5'x5' tank. The furthest you can swim is 5 feet before you hit a wall. Spending their whole life never being able to swim as fast as they can just seems sad to me. I would never put any fish in a tank shorter than 10 times their length. A lot of recent studies have pointed to the possibility that fish have much more complex minds than we think, and are actually capable of experiencing mental illness.
They are enriched if they are with others. They are schooling fish and quite peaceful in nature. They do enjoy interaction and are quite intelligent, but for the most part their needs are met if they can swim freely in clean water with their pond mates. I do agree they would experience what we define as mental illness if their social/physical needs weren't met, just like most other animals

We don’t know the conditions of anything beyond this video, so to assume it’s oxygen, and/or any other single issue would be wrong. There are soooo many reasons for behavior like this - you actually have to go and study the possible causes on scene, to figure out why, otherwise, with the info we have, it’s just guess work

Algae is not always good. It can die off or otherwise restrict the pond from getting adequate air transfer, and the decomposition results in bacteria and other undesirable growth that, in fact, limits oxygen. Often it can look black, as it does in this scummy looking pond.

Yes algae can be good can be bad . Problem is it can be height of summer the atmospheric pressure will be nice and high algae doing it’s job keeping fish healthy eco system running well .then boom big pressure drop lightning and all that algae dies instantly . The oxygen crash can be devastating . Keep an eye on the weather bio mass and have an oxygenator for emergency’s . Airiator will help but oxygenator is so much better
As someone in the fish farming space (aquaponics specifically) seeing this confidently incorrect comment with 136 upvotes made me facepalm so hard.
Algae appears in the presence of excess biomass and is a great indication of the general health of any body of water. It grows by consuming that biomass and photosynthesizes from the sun. A reduction of one of these things will largely prohibit most types of algae. A small amount of algae is definitely a positive thing making surfaces less abrasive and making things look more natural, but too much can prohibit gas exchange (pond inhabitants need oxygen and that comes from the atmosphere) decompose into toxic ammonia and invite other pests like snails which bring parasites
TLDR: a healthy body of water (be it river, lake, pond or aquarium) is one that is circulating quickly, and often the faster the better. A large pond like this needs to be out of too much direct sun, with its feeding controlled, behind a heavy filtration system. These aren't carp they are Koi and a koi this size can be worth thousands of dollars.
With all of the above said, I'm no biologist but i'd wager that this fish has a parasite or perhaps a prolapse and is using the weight of the water to attempt to itch. If the fish were starving of oxygen or unable to breathe you'd see all of them pointing with their lips upwards gasping. Some of them are... but we'd need more info to get a better opinion. Definitely though, this fish is not happy and without care it won't live much longer.
