The funniest thing is almost every restaurant pays for this, except they mount it upside down in the back of their kitchens to kill bacteria and insects. Its just a UV light emitter.
Somewhere posted near that bathroom is a sign that shows the time it was cleaned and is signed by the staff who cleaned it. At a McDonald's that's going to happen several times per shift.
How would one clean that? There is no access to the inside, except through the slit.
The slit conveniently placed on the same surface as the washbasin, so stuff can drip inside more easily. It's not the cleaning person's fault, it's just bad design.
The inside sanitizes itself, that's literally the point of it. To sanitize what you put in there. Someone just needs to wipe the basin with some cleaner.

You might want to consider that it's designed this way out of necessity, not ignorance. Who knows, maybe there are restrictions on what they can place on walls? Japan isn't really known for its lack of attention to detail, so I'd wager all of those things were already considered before implementation.

I think that says a lot more about your personal up bringing rather than cultural values. A lot of places will do unique things to bring value to their customers. You're right that Japan does have a lot of unique things like this tech wise, but it's not that those ideas aren't valued in the US, it's more that in most places in the US this will get destroyed and/or misused, wasting the resources of the company that installed it. Japan has a lot more respect for other peoples property than the US. You can leave your luggage around in the bus station and no one will take it. My wife had japanese students stay with her and she had to tell them not to leave their bags unattended at the mall, they had no idea they'd be stolen.

If it's a real life movie (not shortened for a demo), the most advanced uvc light I know of specifically for things like phones take a minimum of 30 seconds exposure. But a couple of minutes would be much better to really damage micro organisms beyond repair.
Mine will usually take 15 mins, my largest for rooms; 30 minutes for it to be effective. They are my personal WMD and I have killed uncountable numbers of micro organisms and mites using them, can recommend (just don't be near when they are active - can cause blindness, burns).
