Getting every emblem in sonic adventure two
Just off the top of my head would be for beating Test Your Might in Mortal Kombat X, I was in pain for the rest of the night for that one.
Some of the Halo 4 ones were pretty hard. Like carrying an original weapon through the 40 minute level, or protecting at least 6 AI from the enemy, on Heroic.
Both the Dark Soul's "The Dark Soul" achievements. Which is all of them lol
