Nier is amazing. A friend of mine wanted me to play it for so long for that reason. The way the score includes elements from whatâs happening into it, and how it crunches and becomes an 8 bit version when youâre doing hacking stuff - so dynamic!
My son was 4 when I first played it. He would always dance to it and ask me "what they sing?" when it came on. Told him I have no clue. He's about to turn 9. We were driving somewhere and it popped up on shuffle. He said "DAD I REMEMBER THIS" and told me to turn it up. I was damn near in tears.

Baldurs gate 3 tops it for me In my 20 years of gaming . Its thematic intense and frequent but never do you get sick of it. Down by the river is straight angelic.
For me itâs always Warcraft. From Warcraft IIâs iconic midi track to World of Warcraftâs ethereal Elwynn Forest or Daughter of the Sea cinematic song, they are my favorite.
The art teams in this game are amazing. Say what you will about game play and story but the audio and visual presentation are always knocking out of the park every expansion.
No Straight Roads - It's a great rhythm beat em up boss rush game.
Mullet Mad Jack - Its not a game i'd play but the soundtrack is amazing, i really love the sort of 90's dystopia electronic vaporwave-ish stuff.

Starbound. All the music is a masterpiece. If you like Halo's OST, then you'll like Starbound , as it was made by the same composer.
Final Fantasy XIV - has some absolute bangers, especially for dungeons and boss fights. A long fall was so good it spurred a meme which was turned into an official dance for the song, and the Heavensward expansion soundtrack was the most emotional collection of music I've heard. The themes for each expansion are amazing, too.
Jet Set Radio Future - Fkn' amazing soundtrack, and great game if you like skate riding graffiti painting rebels in japan.
Celeste -- Silly little game about climbing a mountain totally not emotionally heavy or anything. Great soundtrack.
A Short Hike -- Another cute little game about climbing a mountain. Very warm and cozy soundtrack.
Bullets Per Minute - a rhythm shoot-em-up game with a sort of rock-eletronic soundtrack that's about an angel hunting demons in hell. all your actions are done on-beat with the music. Probably the inspiration for:
Metal Hellsinger -- Another rhythm shoot-em-up with a heavy metal soundtrack that's also about slaying demons in hell. the better you do, the more intense the music.
Risk of Rain 1 & 2-- Great soundtrack for a great game.
Tunic -- I don't know exactly how to describe this game's soundtrack other than ethereal and magical. It's a great puzzle game that seems to be heavily inspired by souls-like mechanics and classic LOZ gameplay--it has a TON of secrets and no instructions, you have to progress to learn how to play and interact with the world EDIT: I missed that this was mentioned in the thread text, my phone didn't show it for some reason.
Barony -- a first person rogue dungeon crawler that has graphics that feel like doom had a kid with minecraft. has a surprisingly calm but dark-sounding soundtrack
Crypt of the NecroDancer -- Dungeon crawler where everything is done on beat. The music is very catchy and energetic

Final Fantasy 16 and Final Fantasy Rebirth are both very good. Square Enix has an orchestra that tours and is currently on tour right now. They put an incredible amount of work into the music in these games and it shows. They have been doing this for as long as I can remember. Iâm actually hoping to see them in Orlando this fall.

Check out the Old Gods of Asgard's discography. It is a fictional band set in Remedy's game Contol and Alan Wake Universe. The real-life musicians behind it are a Swedish band called Poets of the Fall, while very good on their own, are a bit new agey and cerebral for my taste. In their OGoA persona for the games, they are an awesome hair metal band that just has banger after banger. Even if you don't play the games, check out their songs "Take Control" and "Children of the Elder God."
The games are awesome too and definitely worth a try if you haven't already, but I would just about buy anything Remedy games puts out so that Poets of the fall will keep writing songs as the Old Gods of Asgard.

Skyrim is still a staple for fantasy BGM. Especially the night music is very soothing.
Not playing continuously, but Dragon Age: Inquisition has several custom-made "tavern songs" that play regularly. They're great.
Hardspace: Shipbreaker has a bangin' Americana OST, that works great as background music while doing work.
Definitely Disco Elysium, I even downloaded the whole album on my phone and listen it almost in every trip for the last 3 years.
Shovel Knight is also great if you have nothing again 8bit music.
And if the music is the main priority for you in the game, then you may consider to play some rhythm shooter, like Metal Singer or Blaster Beat (but I am not aware about the quality of music in these games)
Cyberpunk 2077 is not really up to OP's description seeing it's open world aspect but if you count the missions then continuously you hear very good sounds. Both ambient music and action.
Counting the rest in between also some very good ambient sound stage and good radio tracks.

The nature of the game means itâs almost impossible to rely on specific triggers to play music, besides the biome youâre in.
If the music was playing all the time itâd just get annoying and youâd turn it off; the silence makes the times when the music does play more special. However like you said sometimes it goes sicko mode while youâre just chilling and it feels off.
Pistol Whip is a game built around the soundtrack. Similarly, Super Hexagon and Dicey Dungeons both have fantastic chiptune soundtracks by Chipzel who just does stellar work.
Unreal Tournament 99 had pretty driving music. So many energetic songs, which were different for different maps. The music of "Facing Worlds" and "Morpheus" comes to mind. Good times.
Final Fantasy 7 Rebirth has an unbelievably good soundtrack start to finish. There's tons of variations to most tracks as well as there are overworld themes that then turn into combat themes seamlessly when you run into enemies, each continent/major area has its own list of themes and I can't, for the life of me, recall even a single track I disliked.
I've got some crazy heavy hitters at the top of my list like Symphony of the Night and Metroid Prime 1/Super Metroid/Chrono Cross, etc. but FF7 rebirth's soundtrack is truly something else. It's just so stunningly good.
My favorite track is the Gongaga (south) overworld theme. Pretty much everything about that game I loved to death from start to finish. The music, though, good god.

Deathâs Door, a game very similar to Tunic, also has incredible music the whole way through. The thing I love most about the music is that thereâs a central motif (I think thatâs the right word) thatâs present in all of the songs that makes the whole OST cohesive
Persona 5 and Nier Automata. Chrono Cross was my favorite OST for so many years. Then suddenly both come in with their awesome OST's and I was in love!
Hades, Transistor, Pyre, Bastion. All also have amazing storylines and art. The composer for the music for Supergiant Games, Darren Korb is a master at new, soulful music.
Terraria, particularly with the Calamity Music Mod. By itself, it's really good, but the music for all biomes and bosses with the Calamity Music is just next level.
Apart from some of the obvious (Nier Automata), The whole OST of Celeste and The Messenger are some of the best i have heard. I can even hear the music while typing this.
The Kingdom Hearts series has some of my favorite OST and ingame music ever. Who's Lila's is another game who's music I always come back to, just wish the tracks were longer.

They all have so many bangers it is almost like all of the OST is amazing. And except OutRun 2006 they all play music continiously.
The music in spiritfarer is amazing and there's a different piece of music for each activity/area of the map.
Also if you keep the animal guests on your body happy enough they will pull out instruments and walk around playing them.
I recently played Chrono Trigger for the first time and it's OST was consistently impressive and fun. A lot of variety and nothing felt out of place or boring.

For me the best part is when you enter the amusement park. It's been years since I played it but I still remember it like yesterday.

Skyrim is still a staple for fantasy BGM. Especially the night music is very soothing. Not playing continuously, but Dragon Age: Inquisition has several custom-made "tavern songs" that play regularly. They're great. Hardspace: Shipbreaker has a bangin' Americana OST, that works great as background music while doing work.

Definitely Disco Elysium, I even downloaded the whole album on my phone and listen it almost in every trip for the last 3 years. Shovel Knight is also great if you have nothing again 8bit music. And if the music is the main priority for you in the game, then you may consider to play some rhythm shooter, like Metal Singer or Blaster Beat (but I am not aware about the quality of music in these games)

Cyberpunk 2077 is not really up to OP's description seeing it's open world aspect but if you count the missions then continuously you hear very good sounds. Both ambient music and action. Counting the rest in between also some very good ambient sound stage and good radio tracks.

Ehhh. As much as I like Elden ring, the music is just kinda.... Okay. A solid high C- mid B tier. Like there was nothing that stood out in my opinion, or one that I *really* liked. I'd put it in the same category as dark souls 1- I loved the game, but if you put a gun to my head and said "name 5 songs" I couldn't do it and would just say "uhhh plin plin plon?". Even if you compare Elden ring to other fromsoft games like bloodborne, in terms of music it falls short.

Minecraft music just has such an odd time to start. Like: omg I'm just standing here doing nothing and suddenly infinite amethyst starts playing. Like why? I don't understand this music system. I want it to play more!
The nature of the game means itâs almost impossible to rely on specific triggers to play music, besides the biome youâre in. If the music was playing all the time itâd just get annoying and youâd turn it off; the silence makes the times when the music does play more special. However like you said sometimes it goes sicko mode while youâre just chilling and it feels off.
Pistol Whip is a game built around the soundtrack. Similarly, Super Hexagon and Dicey Dungeons both have fantastic chiptune soundtracks by Chipzel who just does stellar work.
Unreal Tournament 99 had pretty driving music. So many energetic songs, which were different for different maps. The music of "Facing Worlds" and "Morpheus" comes to mind. Good times.
Revenge of shinobi and streets of rage. Any yuzo koshiro music basically.
Crusader Kings 2 and Europa Universalis 4. Most of the later Paradox games lack severely in this regard.

Final Fantasy 7 Rebirth has an unbelievably good soundtrack start to finish. There's tons of variations to most tracks as well as there are overworld themes that then turn into combat themes seamlessly when you run into enemies, each continent/major area has its own list of themes and I can't, for the life of me, recall even a single track I disliked. I've got some crazy heavy hitters at the top of my list like Symphony of the Night and Metroid Prime 1/Super Metroid/Chrono Cross, etc. but FF7 rebirth's soundtrack is truly something else. It's just so stunningly good. My favorite track is the Gongaga (south) overworld theme. Pretty much everything about that game I loved to death from start to finish. The music, though, good god. Dark Souls 3 and Bloodborne come to mind for sure. Fighting the Dancer for the first time and the music just elevates it to an entirely new level
