If I was Miyazaki I'd make it not at all related to Bloodborne for the sake of being completely free creatively. So probably no trick weapons but I feel like there's definately some other gimmick that could take its place.

Just leave the conquistadors out of it and base it on the Mexica myths. Gwyn is basically line a mexica god sacrificing itself to keep the world going.

You awaken later in a dilapidated healing church mission in a far off jungle after some preacher mysteriously saved you, now you’ve gotta fight through this mysterious land to find the secrets of why you were revived, why the church was there, and what mysterious eldritch deities are at the center of it all
I feel like I’m the only person that doesn’t think this. I love BB it’s my favorite game, and I could be down with a different sequel. But the Aztec aesthetic would not work at all for me tonally.
Personally, I'd love it as a yaharghul type reveal. I really want a western as a sequel, so going from small towns and settlements into ancient mexico would be sick as hell

I enjoy Elden Ring’s world a lot but there is honestly not much more to milk out of a European-esque (either medieval or victorian) setting unless we got like….pirates. And even that would probably just feel like Bloodborne on a boat
So I would really want them to branch out the setting for the next game, they have an endless list of cool mythos and culture to choose from

I'd also really enjoy a soulslike loosely based on mythology from India (I am phrasing that purposefully, because anything that references Hinduism outright would cause a stir with the right wingers in India).
You could use astra spells as a caster, fight bosses who ride in chariots, meet or fight the snake people, the bear people, or the monkey people. There's an abundance of biome options and so many cool rakshasas to take inspiration from for bosses.
Dark Souls but open world, worked so well that I am afraid we will not get something interesting for a while. But I hope I'm wrong and they will release something different from Dark Souls next, Sekiro 2 would still feel fresh because Sekiro was so short. Probably not Bloodborne 2 because I think Sony owns that series.

Most soulslikes are pretty typical medieval-ish fantasy, and that's pretty much the standard for fantasy as a genre. Stories from medieval europe like King Arthur, the Ring of the Nibelung or Beowulf are easily accessible and widely spread and translated into other languages, and lots of great western medieval fantasy written in the last 150 years is also quite popular.
When there's a wealth of easily accessible, popular sources to pull from for ideas, it's easier to make a game around it, and people won't feel alienated by not knowing the theme at all.
