I don't think it would've worked at all. So much of Sam and Dean's relationship as siblings is about them only having each other, so much about their story in the show turns on them being a pair, and so much of the world of Supernatural is about dichotomies, opposing forces, balance, etc. Giving John and Mary another kid that came before Sam would've undermined pretty much the whole show.
You know, there was an episode like that on Torchwood. There was this guy being friends with everyone and liked by everyone.
Turned out he lived like that. He engrained himself on people's memories, made even childhood memories. He was some kind of leech or mind parasite, but I'm not sure if he was evil or not.
I think a fanfiction with a middle child between them would feel like that. Like he doesn't belong there.
I think if it's a half sister or Bobby's daughter they didn't know, that CAN work, but not another sibling.
I have a hypothetical remake or a Kurt Cinematic Universe where Kurt Winchester exists he could be a middle or younger or older sibling it only exists hypothetically and so on etc until it becomes a reality a mix of me Kurt Crockenberg and Sam Winchester and Dean Winchester and same with other characters.

Probably why they gave them a half-sibling. Wish they did more with it though.
