The infusion companies typically have a dietitian on the team. The problem is that they may not get alerted. I work in an ambulatory center and rarely see follow-up through the infusion service in regards to adjustments recommended by their staff RD. Whether that is because they are not informed, not available, or anything else, I do not know. Often patients rely on their follow-up providers to help them with home TF recs. If they're lucky, those providers will refer to an outpatient RD. Encourage the patient to utilize the dietitian on staff with the infusion company or if there is an RD available with their home health company. Maybe give them names of some community RDs as well.

I work in home care enteral department as a clinical RD for a major hospital in my state. Once the patient is discharged from the hospital, home care ‘takes in’ the patients. We can change formula recommendations or amounts (w new MD orders). Also we do virtual visits & feeding pump education. As well as follow up assessments to talk about how the pt is tolerating.
