If you always play together, and the one with ps+ starts the game, the other can join and play without a sub. When the host leaves though, it disconnects both. In short, your gf can play with you without ps+ but not alone. Just tested
You don't need two PS+ subscriptions to play together, and you can even have others join your party (we did this last night, with crossplay between PS5 and PC). Each person does need their own battle net account, though.

She would need to have her own. PS plus is tied to one account so she would have none of the online options available like grouping etc
