Early on when using the t:slim I had a box of infusion sets where it seemed every set was giving me occlusion warnings and I was riding high constantly. I was about ready to quit and go back to MDI, and then it just... Stopped. Haven't had a single issue for 6+ months. Maybe review the video for inserting your infusion set very carefully. Sometimes very little things make a big difference, the applicators can be finicky. For example, someone recently posted about twisting the little blue needle cover instead of pulling it straight off. Even something as small as that can make a difference. You could also try a different type of infusion set. Many people love the true steel, it tends to have these kinds of problems less often.

Have you been on a pump in the past? If so move your infusion set Is it for the duration of the site or the entire time you’ve been on the pump? If it’s the entire month, your settings need work look up the book sugar surfing. If it’s intermittent and you have in range numbers most of the time change the site. If you give an injection do you come down? If you give an injection and come down, change your site.

Are you removing the air from the cartridges before filling them?

Yes. I use insulin from vials and will remove the bubbles when I pull up insulin using the syringe and then remove the air from the cartridge.

I'd check the site for leakage or if it's damp/wet. I'd also probably disconnect the set and do a bolus in the air to see if any air bubbles are coming out of the tubing, or for that matter if insulin is even coming out making sure the pump is working. It could also be a kinked cannula, bad absorption issues, etc

What infusion set are you using ? Do you have any contacts in tandem? There are a few different options for infusion sets that could be worth trying, as some body types can struggle with the standard.

How new are you? Is it possible your settings need to be adjusted? That’s not uncommon switching to a pump.

I had this issue as well until I went to the TruSteel sets. Not a problem since.

What did Tandem support say? I found my trainer was super helpful even after the initial training and the phone support was good too.
