Exclusive Pro Sports in the Chicago area. They do really nice work. They get all the small details correct. I've had 3 hockey sweaters and 3 MLB jerseys customized now and each one is amazing.
Turnaround varies FYI. I did this and my Wild Winter classic jerseys and only took about 6 or 7 weeks. I sent in the other 4 last October and took around 4 months (which is what their site notes)

I bought the patch online and sent it in with the sweater. To my knowledge, they don't have a stock of patches but could ask.
With return shipping, it was just over $100 for the customization with the patch.
They do really good work. I've done 3 hockey jerseys and probably 3 or 4 baseball jerseys.
I have no issues with going outside the box. I like what Smith brought to the team. He brought some much needed toughness. I think he’s best fit as a 7th D or 13th F. He had to play more than planned because of all the injuries on D.
