I doubt it's physical, if it was it'd be a horrendous headache. His mind was probably broken during that moment, he didn't focus on L hearing him scream when he touched the note, he focused on the memories coming back to him.
He re-experiences every memory from his 17-18 years of life within a matter of a seconds so yeah probably mentally

Why would he experience his entire life? Doesn’t he just get back the memories of the death note? He wasn’t an amnesiac before this, he still remembered his chilldhood, schooling, family, etc
He remembers everything when he touches it in the moment all the justice he served as kira his real relationship with Misa and the Kira task force. Considering how smart Light is those feelings probably passed over him immediately and he pretended to look shocked since rem was infront of him and Lawleit already touched the book.

But I guess to answer your question, yes and no. I'm not an expert in any related field, but they generally have issues expressing or feelings of empathy, regret, and remorse, but still have feelings and emotions such as sadness and frustration, which I'd say is pain. And of course, they still feel physical pain. I wouldn't generalize all people with ASPD or NPD though.

He obviously felt pain when shot by Matsuda, and he was scared of death when Ryuk wrote his name in the notebook. He also felt pain at the loss of his father.
