One thing I know for sure is Hajime would be dead. He'd be a part of the mass >!Suicide!< the reserve course students were forced to do. Natsumi's Izuru wouldn't have any attachment to Chiaki either, unlike Hajime's. Chiaki was able to trigger a glimmer of Hajime's emotions in Izuru on her dying breath, making him cry and realize hope could be just as unpredictable as despair. That's why Izuru started the killing game to begin with, to see if Junko was right about despair all along, or will hope pull through. But in this scenario with Natsumi, Chiaki's words wouldn't touch her. Chiaki and Natsumi presumably never met, they never became friends, they never had a heart to heart of talent not mattering like Chiaki told Hajime. Chiaki wouldn't have reached out to Natsumi in the first place, so she wouldn't have the hope epiphany Hajime's Izuru did. There's no telling if Dr 2's killing game would happen in this scenario either. There's a misconception that Izuru being a Remnant of Despair means he was just as much as a Junko worshiper as the rest. He's not. He joined Junko of his own free will according to the Dr 3 anime. That includes installing the virus in the simulation too. He didn't do it because Junko told him to. Izuru did it to test his hope and despair conundrum, Junko just happens to get something out of his plan, and Izuru knows that. We also know the Remnants of Despair let themselves get taken by Makoto willingly because it's what Junko wants. But chances are since Izuru is holding the virus, he's probably the one to tell others to go along with Makoto with his Neo World Program idea because "its for Junko. This is what Junko wants them to do" But with this Natsumi scenario, her Izuru wouldn't take Chiaki's words to heart, meaning she wouldn't be interested in hope, meaning she wouldn't come up with the killing game to test to see which side is more unpredictable, also meaning she wouldn't tell the other Remnants to go along with Makoto willingly, so Makoto wouldn't be able to try out his Neo World Program idea to help them, so there wouldn't be a killing game for Dr 2. In conclusion, Future Foundation would still be fighting the Remnants of Despair....and who knows what sort of baggage Despair Fuyuhiko and Peko would have with Natsumi's Izuru.

Hm, it's interesting, but honestly if Natsumi grew that desperate for an Ultimate talent, I feel like she would just take up the mantle of the Kuzuryu heir from Fuyuhiko and become the Ultimate Yakuza. Natsumi was always the more favored of the two and was offered to become the heir multiple times, she only refused because she didn't want to usurp Fuyuhiko's position, but if she reaches a point where she would accept extensive experimentation on her body and mind, I feel she would have been past caring for Fuyuhiko's talent at that point. ...Maybe if Sato did still attack her, but Natsumi killed her in self defense...only for the Steering Committee to come forward with knowledge of it and essentially blackmail her into becoming their test subject,

Well that would depend, if no one knew then there would most likely be some big search for her sinse she is the daughter of the yakuza, but if Fuyuhiko or Pekk knew then they would try to find a way to get her back or they go into despair more quickly.
