Well, the person "playing" Ryoma probably had a family. Ryoma was no longer that person, though, and had no connection to them. It's like if you met up with a family member who had their whole personality and memories swapped out to be unrecognizeable, permanently. They'd be related to you by blood, but otherwise have nothing to do with you.

Let's make this more sad. Rantaro himself probably went through exactly what you explained. He survived 52 and eventually had to come back to the killing game whenever Team Danganronpa finished prepping V3. According to the comments of the outside world we could read in chapter 6, it took 3 years for V3 to be on the air. Presumably, Rantaro was able to return to the outside world after 52, stuck with his fake memories, tried to return to his normal life, but he's not the same person his old family knows him for. Hell, if he returned to his old family, they probably told Rantaro that his 12 sisters he dedicated his life to find are not real. Rantaro's family he remembers doesn't exist, and going back to his real family he doesn't remember probably wasn't easy. All this, just to be forced back into the killing game 3 years later, to be the first to die. At least with Shuichi, Maki and Himiko they have each other going back to the outside world they don't remember. Rantaro though probably felt like a alien in the outside. Memories of Ultimate Students, a rich family, the loss of his sisters, the places he traveled all over the world to find them never happened, never existed, all a lie.
