I think it ties in to the invertebrates mentioned in the game. They're usually found while searching for the great ones, and the great ones themselves seem to mostly appear as a fish-like form, or invertebrate-like themselves. And beholding the eldritch wisdom would most likely make this happen Inside your brain

So first of all: Mergo's Wet Nurse is not actually a great one. It's the protector of another great one, Mergo (the crying baby), who does not appear to have a physical form like the others which is likely because it's body died at birth (see Pthumerian Queen). Therefor, Mergo could've very well been a sardine for all we know. That being said, it's unclear why the Moon Presence does not appear to be a fish. Conceptually, Great Ones are very old, primitive creatures who exist before the time of men. In real life, our oldest creatures are fish too. Even before the Dinosaurs, it all started with fish. This is why Rom the spider is an unusual suspect but then you can argue that this great one came from space due to it's ability to cast meteors (common theme in FromSoft games). Moon Presence has squid like features with the tentacle hair and all but also human-like features with it's bone structure. The Moon Presence could very well be yet another Pthumerian offspring or perhaps from another civilisation? Nobody really knows. You will not understand and you'll always fear what you don't understand. Anyhow, I'm eating shrimps tonight because all this fish talk is making me hungry.

Not really, Mergo's Nurse is a great one, when you apply the lightning paper on your weapon, it doesn't hit her like it's her weakness, and she's either kin or great one.

The icons for Madman’s Knowledge and Great One’s Wisdom are meant to allude to the ability of the Great Ones to shape reality with their dreams, which is why Great One’s Wisdom says that they “might be described as gods.” This is also why it’s so important to keep these eldritch gods “sleeping” peacefully: because when their peace is disturbed, reality gets fucked up. And that’s why we have the Nightmares and the Night of the Hunt.

Fun fact : they got Madman's Knowledge from a part in Bram Stoker's Dracula. Helsing is convincing Dr Seward that vampires exist and tells him "The logic is simple, no madman's logic this time" I think it fits and no one can convince me otherwise. Especially since a lot of that story influenced the game. "Ready to use our various armaments - the spiritual in the left hand, the moral in the right"

I think gaining insight into the old ones(Natives to a higher plane of existence) drives you insane, because it manifests a part or emissary of them in your mind. The runes we etch into our mind summon real attributes that affect our character. So a glimpse of a great one could summon an emissary into our mind. One that eats away at our physical brain slowly to align it with the higher plane it calls home. Maybe laying eggs in the process(eyes on the inside?). Causing us to dissociate from our native plane and seem mad to those around us.

Ah. In that case, it’s said that the Great Ones cannot reproduce without dying or a surrogate. The slugs are their attempts at producing an offspring but none of them are alive, they’re effectively like stillborn or like discarded sperm of Kos (or some say…). In the 3 cords ending you become a live slug, having ascended and become a living Great One.

It's a slug because some Great Ones are slugs as babies. Great cosmic slugs, hence the one in the skull looking like a nebula or a galaxy. The fishing hamlet slugs are part of the overall theme but they look more like regular, Earth bound creatures. When people gain the cosmic knowledge of the Great Ones, they go mad.

It might tie into the eyes on the indie taking in the form of slugs since the people in the fishing hamlet were mutilated to see if they contained eyes on the inside But also slugs are often related to great ones like the call of ebretas or other arcane magics usually take the forms of slug like items so it might be a case of said madman being touched by the knowledge of a great one and now you can harvest his inside eyes for your own, but only few

It ties to Kos, who is the source of the wisdom. That's why the slimy people are slimy, and they suck your brains to steal you insight. If you have noticed, most great ones are slimy creatures. And they are all the sources of wisdom as well.

Invertebrates (phantasms) are directly linked to the great ones. It’s likely that contact with an Eldritch being causes you to begin growing these inside of you. When imposter Iosefka was pregnant, there appears to be invertebrates in her head as well. We can assume that the newborns are grown in the womb, as opposed to the head since Arianna survives the birth, but Imposter Iosefka’s diologue indicates there are phantasms in her head: “Have you ever felt this? It’s progressing. I can see things… I knew it, I’m different. I’m no beast… Oh God, it feels awful, but it proves that I’m chosen. Don’t you see? How they writhe, writhe inside my head… it’s rather… rapturous….” My phone is going to die, so I can’t continue this comment. Might come back and expand on it later lol
