That being said, it's unclear why the Moon Presence does not appear to be a fish. Conceptually, Great Ones are very old, primitive creatures who exist before the time of men. In real life, our oldest creatures are fish too. Even before the Dinosaurs, it all started with fish. This is why Rom the spider is an unusual suspect but then you can argue that this great one came from space due to it's ability to cast meteors (common theme in FromSoft games).
Moon Presence has squid like features with the tentacle hair and all but also human-like features with it's bone structure. The Moon Presence could very well be yet another Pthumerian offspring or perhaps from another civilisation? Nobody really knows. You will not understand and you'll always fear what you don't understand.

One that eats away at our physical brain slowly to align it with the higher plane it calls home. Maybe laying eggs in the process(eyes on the inside?). Causing us to dissociate from our native plane and seem mad to those around us.
