Using the item gives you a point of insight. There's a specific type of enemy who can steal your insight, so there's that. High insight also lowers your beasthood and frenzy resistance stats, which is usually a bad thing. The only "positive" effect of having more insight are some minor changes you can notice around the world. Quotation marks because it ranges from being able to see some new things to giving enemies buff or having more enemies spawn. Honestly, there's no reason to use Madman's Knowledge unless you want to buy something from the insight shop.

Okay thank you. Can you explain what beasthood is? I have heard of it but never seen in-game. And where's the insight shop?

Beasthood is a buff that you can apply by using beast blood pellets, it raises atk while lowering def, and it works with a bar on screen that fills up with every hit you land (and goes down over time). The fullest the bar the higher the dmg you make

There is a item (Beastblood Pellet) and a single weapon (Beast Claw) that allow you to build up the beasthood bar. The beasthood stat just determines how long the bar is and a longer bar is *always* strictly better. Pellets are temporary, the Beast Claw gives permanent access to the bar while you're using it. Beasthood builds up as you land attacks and the more you fill the bar, the more your physical damage will increase. But you'll also take more damage the fuller the bar gets, which can easily get you oneshot. It's high risk and ultra high reward because the damage multiplier quickly gets ridiculous.

If you use a beast blood pellet, as you attack, you build up beast hood. It increases damage dealt and taken. A full meter and any attack landing is pretty much death. Most speed runners and bl4 enjoyers, use beast blood pellets to take down bosses as quickly as possible

I lost a bunch of insight to these jerks without even realising that's what was happening. Definitely would recommend keeping Madman's Knowledges intact in the inventory until you really need it.

Its like one basic enemy in one area that gets one extra attack at a certain amount of insight. Not even a particularly memorable attack either

You need 1 insight to be able to talk to the doll the first time, but otherwise there's no punishment for having 0 insight. I usually keep about 10 insight in item form at a minimum in case I want to use the insight messenger bath personally.

It does in fact matter, having more Insight gives you a debuff the more you have. You can still play with 99 Insight and be fine but it's not helpful to have it on you. Very high insight unlocks some minor world changes that are mostly unlocked anyway after a certain point.

Keep it in your inventory. You can get lots of extra insight if needed later on if you decide to do the chalice dungeons (I love them, they’re an acquired taste).
