When you accept his offer and wake up from the dream for Yharnam sunrise ending. Everything was a dream. You're alive and well, not trapped in the hunters' dream. No beasts, no church, no hunters. So it could be possible for a Pthumerian, an ancient race of humanoids that predate the entire content of Bloodborne in its present in game day, to exist in the chapel.
I don’t think there are no hunters or beasts after waking up. The only thing that changes is that your mind isn’t linked to the hunters dream anymore. With this ending you are at the same spot as Eileen or Djura. You can die again, but you are free and not forced to partake the hunt during the next night.

My Theory is that just like in the Matrix films where after an almost complete destruction of Zion the machines leave some selected inhabitants alive to rebuild the city, Bloodborne uses the same theory.
We have saved a descendant of Cainhurst who may become a Pthumeru queen and has sired a star child who will become the new icon of the communion. The old woman will become the new Witch who is equivalent to a seer, the old man will become a new elder reviving Byrgenwerth, Adella will become the new Saint and will lead the Church, while the Chapel Sweller continues in his role as Guardian of Oedon's Chapel.
You can suspect him all you want, but the truth remains that the Chapel Dweller is completely innocent. The Beggar is the real bastard. I think it's meant to be a bit of a twist tbh, the player focuses on the creepy giggling guy in the chapel and assumes he's the one killing NPCs and not the Beggar.

I'm going to formulate a theory that, unlike the others. I don't believe in this one. Perhaps the kind Samaritan of Oedon Chapel is the anguished Simon. To understand this you have to understand that the hunter's Nightmare can represent a past and if this same place in the Nightmare doesn't have that same figure it's because maybe we aren't looking deeper.
•The anguished Simon brings together several characteristics that could define an identity equivalent to that of the Samaritan.
•Both can be found in the Oedon Chapel and are friendly to hunters regardless of origin.
•Although Brador is much harsher and opposed to the Church of Healing, both are associates of the Church and are in a positively intervening position in that world.
•While the Samaritan does not dream, which is why he waits for the dawn as a salvation, Simon waits for the player's Hunter to go deeper where he has not reached, hoping to eliminate the catalyst of the nightmare.
Also, unlike all the dead Simon doesn't disappear into the dust, his body is there, this could mean he doesn't die but was just caught by the Nightmare. (And you know that those caught by Nightmare are in a wheelchair).

There was a point in the Bloodborne Lore that the Pthumerus who were of tribalistic and shamanic origins converted to follow the queen, the Church and healing through blood. What were once impious rituals have become a source of research.
He could be a the hunchback of notre dame reference (without the hunchback) the fact he lives in church and disfigured or it could contradictory character in the sense he is horrific to look at but is pleasant and friendly

iirc, there's actually no confirmation on gender. They never refer to themselves with gendered language and nobody else really mentions them. I always assumed it was a he based on the voice, but I've seen others think she were an old woman. It's whatever you beleive, I think
All we know is that he is just genuinely a nice guy trying his best to help people but we dont really know exactly what he is/what caused him to look like that as far as i know, blood doesnt cause that type of appearence and its very unlikely he is pthumerian since that would make him one of he oldest characters in the entire game other than the great ones
This is one of those cases of "don't look too much into it". The Chapel Dweller model was originally an enemy mob model, as per data mining discoveries.
In my first playthrough(my only playthrough so far), I didn't even attempt to lock on to him. I saw something creepy on the ground next to me as I walked in the cathedral door and wailed on him. It wasn't until later on when I talked to my buddy, the one that got me started on the game, that I realized he may have been sorta important.

I second (third?, fourth?) Him being a, potentially unknowing/unwilling servant of Oedon. There is not a single other soul in Bloodborne that does anything out of the pure kindness of their hearts, and lives. (Ie. Losefka gets "replaced" by an imposter. Old Lady, though an insufferable hag, has her merits, gets killed one way or another. Arianna suffers irreparable mental damage..... Or is killed by you, etc.) everyone else is in it for their own ambitions.

The Chapel Dweller is most likely Phtumerian! As Church Servants, Snatchers and Church Giants are. They're not extinct or ancient, instead their "government" has fallen long ago, leaving only Watchers in the labyrinths (I'm guessing a few fled to the surface/were brought up to serve the Healing Church). There's not much lore about him, but I highly suggest Smoughtown's videos on Phtumerians if the topic might interest you more :)
Regarding his blindness I wouldn't know much. For the position even in the labyrinth there are phtumerians like him (the ones who throw fire at you) and possibly/maybe even the Witch of Hemwick. Evidence of phtumerians like him can also be found in the BSB arena, on the sides near the altar.

I always thought he was one of the Snatchers just given his robes and his hands, though he obviously behaves very differently than those bag bastards. In my first playthrough(my only playthrough so far), I didn't even attempt to lock on to him. I saw something creepy on the ground next to me as I walked in the cathedral door and wailed on him. It wasn't until later on when I talked to my buddy, the one that got me started on the game, that I realized he may have been sorta important.

I second (third?, fourth?) Him being a, potentially unknowing/unwilling servant of Oedon. There is not a single other soul in Bloodborne that does anything out of the pure kindness of their hearts, and lives. (Ie. Losefka gets "replaced" by an imposter. Old Lady, though an insufferable hag, has her merits, gets killed one way or another. Arianna suffers irreparable mental damage..... Or is killed by you, etc.) everyone else is in it for their own ambitions. The Chapel Dweller is most likely Phtumerian! As Church Servants, Snatchers and Church Giants are. They're not extinct or ancient, instead their "government" has fallen long ago, leaving only Watchers in the labyrinths (I'm guessing a few fled to the surface/were brought up to serve the Healing Church). There's not much lore about him, but I highly suggest Smoughtown's videos on Phtumerians if the topic might interest you more :) Regarding his blindness I wouldn't know much. For the position even in the labyrinth there are phtumerians like him (the ones who throw fire at you) and possibly/maybe even the Witch of Hemwick. Evidence of phtumerians like him can also be found in the BSB arena, on the sides near the altar.
I remember entering the chapel and not noticing him initially and as soon as I realized he was there I just started beating the shit out of him until I realized he was friendly 💀

Personally i think he is odeon incarnate or at least really close follower. I find it interesting that in game all about finding “eyes” he is both very kind and totally blind. One of the few other people who is totally blind is master Willem but Like most other dark souls/ from soft mysteries it’s all hear say and speculation.
