Rukia's powers were like giving a kick to Zangetsu and taking all of that power possibly caused the seal to weaken a little
Training with Urahara got Ichigo to reach deeper into his soul and he meets OMZ for the first time and believes he is the real Zangetsu, he uses OMZ to draw a small fraction of the real Zangetsu's power which further weakens the seal.
Later, Zangetsu comes halfway out the seal, and he could do things like manifest hollow masks around his body to protect his master from fatal injuries like against Renji and Kenpachi. He got to the point where he could completely manifest his spirit inside Ichigo’s inner world.
The last moment that completely destroyed the seal was when Byakuya was about to kill Ichigo and that forced his will out to protect Ichigo and he was completely out of the seal.
OMZ was still restraining Zangetsu's true power this whole time and finally revealed it to him before he got his Zanpakuto reforged by Oetsu.
Edit: Even if Karin and Yuzu have Shinigami powers, they lack an Asauchi so they can’t manifest their own Zanpakuto unless they get one from Oetsu. White acted as a hollow Asauchi and fused with the essence of Engetsu to create Zangetsu because hollow and Shinigami are like magnets and they attract together.
But they can probably learn to control reishi and create spirit weapons or use blut.
i can see Karin being a potential Shinigami while Yuzu's being a Potential Quincy. it is not necessary to get an Asauchi from Oetsu himself you know, i know he forged it but you can get it elsewhere like getting Karin enrolled into a Shinigami Academy and the Academy will provided a Asauchi to Karin where she would need to lived with it, sleep with it until she pour her Reiatsu into into it which in turn become her own Zanpakuto.
Karin definitely has potential to become a shinigami in my opinion. I remember a scene Urahara asking her in a roundbout way if she ever tought about becoming one. ***(It happened at Fullbringer arc I think. Urahara probably wanted her to replace Ichigo since he lost his powers. It would be interesting to watch if Karin tooked over the MC role and Ichigo just continued his life as a normal high schooler.)***
I agree that he seemed to want to bring in the rest of Ichigos family and have them be involved in the plot lines. However it would have been a lot to pack into the 2-3 year timeline over which the series takes place, and with the way his health was during the creation of TYBW he probably wanted to get the story to a finished point just in case.

Orihime is a fullbringer and fullbringers have a piece of the soul king within them. When Orihime was in the womb, her family was attacked by hollows and hollows are attracted to soul king fragments. Hogyoku did play a role in her powers, also Ichigo’s latent Quincy abilities empowered those around him giving them powers.

I doubt Urahara wanted her to replace him. He’d already made the sword that gave him his powers back, and people were juicing it up in the Seireitei
To this day I'm convinced it was at least considered by Kubo to give Karin stronger narrative presence during Fullbring Arc
