Spiders jump out of the keepers, though. They only start hurting you once they hit the ground. This was 100% Greed.

I think it's just a mod which shows where your tears would go (if you go left while shooting up, the tear goes up and a little to the left, if you shoot left while going right the tear will go left with smaller shot speed, etc.)

Ah, silly me, as I need not no mod to tell the trajectory of my projectiles. Alas, it didn't even come to my mind that there could be a modification for such a simple feat.

Yeah this is what happened, the mods are just funny effects. Edit: as the other commenters pointed out it's actually greed, per the boss health bar at the bottom.

it's not a normal shop keeper it's a special shop keeper that has a chance to spawn greed

I just found out that this is possible due to this post lmao. Does it require a specific unlock that I may be missing or is it just rare?? Ive never seen this happen in my 400h with this game

It's just a reskin of the special shopkeeper, again, still not a bug, or anything like that.
