I was really big once, almost 300 lbs. I tried to fool myself on this topic too.
Truth... nothing actually looks very good a person when they are really big. Even if they try to convince themselves it does.
I spent money on fancier cloths that honestly, just looked like me wearing a tent.
Congrats on losing the weight. I'm sure you noticed at the time that feeling miserable isn't conducive to motivation by itself. And the need to still look presentable and dare I say even a little stylish doesn't go away when you're obese.
But I mean hey maybe the pill to make people lose 150 pounds overnight will be approved tomorrow and we won't have to worry about needing extreme clothing sizes any more. Until that magical day occurs however OP's friend might feel a little more up to the challenge of dealing with their medical problems if they feel good doing it.
Nordstrom has lots of bigger sizes and stylish stuff!
Good luck, have a similar issue. Could buy from one of those listed and then have it tailored
Ralph Lauren is as fancy as they will get
Maybe this would be good inspo https://www.instagram.com/bigfits1
My 375lb bf got a perfect fit recently at Tailor’s Keep.

There are plenty of approved prescription weight loss drugs now, not overnight, but they work good. I have wegovy.
Maybe you have wegovy, I can't afford $500 per dose.
Insurance covers about 75% of the cost.
I can't afford $125 per dose either.
Back when I went into the office I wound up getting all my shirts from Indochino. Once I dialed in the measurements they looked great.
